**Please note that all New Patient Assessment sessions must be paid in full before the appointment.
Terms and conditions for treatment
- All patients will require a New Patient Assessment appointment. Full payment will be taken at booking for New Patient Assessment.
- You will be asked to complete a medical form and detail any medications taken, allergies, and current and historical relevant medical conditions/history. Patients will not be treated without completion of the medical form and consent form for treatment.
- Any information provided is private and will not be shared with any other parties without patient consent.
Cancellations
- 24 hours’ notice is required for cancellation.
- Any appointment not cancelled within 24 hours must pay the full appointment/consultation fee. Exemption due to exceptional circumstances may occur such as bereavement, exceptional illness, or extreme mitigating circumstances but must be agreed by Christchurch Foot Clinic.
- By booking an appointment you agree to your cancellations policy.
- Emergency appointments are available upon request, subject to availability.
